In the city of Fairlake, West Virginia, the HillickersThree Finger, Saw Tooth, and One Eye start a killing spree with the help of their grandfather Maynard Odets, who is also a serial killer.
Meanwhile, five friendsBilly, his girlfriend Cruz, Lita, her boyfriend Gus, and Julianare going to Fairlake to attend the Mountain Man Music Festival for Halloween.
On their way, they almost run over Maynard, who attacks them. They fight back but are arrested by sheriff Angela Carter and her deputy Kevin Biggs.
While Carter takes them to the police station, the cannibals kill Biggs and cut off the phone and power lines to the town.
The friends register at a motel while waiting for Billy to be released by his lawyer. Three Finger kills Cruz when she goes to visit Billy at the station.
Gus is abducted by the cannibals while One Eye tries to capture Lita, but she escapes. Carter sees Gus being dropped by the cannibals with his legs broken. She tries to save him but they run him over with their truck.
Carter frees Billy and gives him, Julian, and prisoner Mose shotguns to guard the station. She goes to an appliance store to contact radio jockey Teddy for backup, but he thinks it's a prank.
Lita reaches the station but Billy and Julian leave to look for Cruz. The cannibals surround them with traps and kill Milla with an arrow.
Jen, Adam, Darius, and Luis are captured by the cannibals and taken to their settlement in the forest. They are put on trial by Maynard, who is their leader.
Maynard sentences Adam to death for killing one of his sons. He also sentences Jen to be his wife and Darius and Luis to be his slaves.
Carter arrives at the settlement and shoots Maynard in the eye. She frees Jen and Darius while Luis fights One Eye.
They escape but are chased by Saw Tooth and Three Finger. Carter kills Saw Tooth with a shotgun while Jen kills Three Finger with an axe.
